Your Unique Identity (for use during client intake process)
There are so many components that make up our identity. Some of these things can be seen, some cannot be seen. some of these things are part of our present experience, and some of these things are part of our past but still impact our identity (e.g. perhaps we were previously married but now we are divorced. or perhaps we were religious but now we are not).
Clients: Use this list as a prompt to share any identities that may not be visible or may be part of your past, but would be important for your therapist to be aware of.
Therapists: use this list as you are getting to know your client to make sure you have taken the time to better understand all the factors of your client‘s unique, complex identity.
